Just sign in your old account, go on the playstation store then on download history in the top right and you can re-download from there

account management > transaction management > downloads list
you can only have your account on two activated systems now ( formerly 5 ) so should call sony at 1-800-345-7669 to report your console as broken , if you can still access it sign into your account and deactivate the system from account management
if you can’t you will be able to use a new website sony is creating to deactivate remotely or ask the sony representative to do it when you phone
if you don’t deactivate the system then your two activations will be used up and you won’t be able to download the content anymore if this one dies

Here’s your answer. Sign into your PSN account on your new machine and navigate to account management > transaction management > downloads list.
